I sent your company a reinvestigation request on (fill in this date) regarding certain inaccurate items on my credit report. A copy of my original request and proof of receipt by your company is enclosed as an attachment.

In accordance with the Fair Credit Reporting Act, 15 USC section 1681i, your company is required to complete this reinvestigation within 30 days of receipt of my original request. This 30-day period has now passed, and I have not received any response from your company. I believe that your company has been unable to verify the inaccurate information contained in my report, which is why I have not yet received a response.

Please immediately remove the attached inaccurate items from my credit report. Also, please send a corrected report to me and to anyone else who has requested a copy of my credit file within the last six months.

I am sending a copy of this letter and the enclosed attachments to the Federal Trade Commission.
